Enrollment Agreement
Between C.A.R.S. Network and the Participating Church/Non-Profit Organization
1. Parties
C.A.R.S. Network – “Christ Is Able to Restore Souls,” a faith-based nonprofit ministry ("CARS").
, a church or 501(c)(3) organization wishing to participate in the CARS Network Auto-Repair Assistance Program.
2. Purpose
This Agreement defines the responsibilities, resources, and financial commitments that enable Partner to submit vetted auto-repair assistance requests through the CARS Network portal and serve individuals in need within its community.
3. Partner Obligations
Partner agrees to the following responsibilities:
Follow all CARS Network intake, vetting, and verification procedures as provided in the Case-Manager Training & Policy Manual.
• Summit all inquires and all case reviews through C.A.R.S platform.
Appoint dedicated personnel to serve as case manager.
Offer an introduction of C.A.R.S platform presentation to members of the organization.
• Pay the selected engagement level’s online access fee in advance for a 12-month term.
4. C.A.R.S. Network Obligations
CARS will:
Supply intake forms, training materials, and portal credentials.
Review submissions within 48 hours and communicate decisions (Approved / Pending Info / Denied).
Coordinate repairs with partner shops and track progress
Auto Repair discounts including parts and labor per engagement level.
5. Fees & Payment
Partner selects one Engagement Level and pays the annual fee online. Fees are non-refundable.
6. Term & Termination
Initial Term: 12 months. Auto-renewed unless 30 days’ notice given. May be terminated for breach or misuse.
7. Confidentiality & Data Protection
Partner must keep client data confidential and only use it for ministry purposes.
8. Branding & Publicity
Partner may use branding only for authorized purposes.
9. Entire Agreement
This document represents the full agreement and overrides prior discussions. Amendments must be in writing.
